M25 Northbound exit onto roundabout

The signage and road markings for this roundabout are confusing or maybe wrong. The sign on the exit specifies 4 lanes, the left going A12 West, the middle two A12 East, and the right Lane going to the A1023. Traffic enters the roundabout, left lane goes left onto A12 slip road and the remaining 3 lanes join to 3 lanes on the roundabout. However the road markings for the outside lane read M25, causing all the of the A12 traffic to try and cram into the middle lane. Then as traffic follows round to the on roundabout traffic lights, the 3 lanes are marked A12 for outside and middles, and A1023 for the inside lane. Traffic fro A12 should make use of 2 lanes all the way around, but due the signage they merge and then split again, causing traffic. To rectify this issue the road markings should be repainted, and tracer lines should be added to shepherd traffic into the correct lanes. This would reduce the volume of traffic at this roundabout. Atm even during quiet times, there is often a queue caused by the confusion, and I have seen numerous accidents and near missed caused by people panicking and jumping between lanes.
